lead Entity for the Continuum of Care

CHIP - Coalition for Homelessness Intervention & Prevention is designated as the Lead Entity for the Indianapolis Continuum of Care (CoC). At the heart of a collective impact structure, the Lead Entity is empowered with dedicated project management staff and technical experts to implement coordinated community efforts to prevent and end homelessness. The backbone of implementation comes from holding and communication the vision for all stakeholders and creating a culture of transformative learning, evaluation, and adaptive programming. CHIP, with support from the City as the Collaborative Applicant, will utilize a strengths-based approach and work in tandem to create an infrastructure that maximizes capacity and works to reinforce the goals of collective impact and implementation of the Indianapolis Community Plan to End Homelessness.


CHIP's Responsibilities:

  • Convening and facilitating the Blueprint Council and key working groups

  • Monitoring strategic coherence across efforts

  • Coordinating communication within the CoC

  • Managing collective data systems (HMIS) and information distribution

  • Mobilizing planning efforts that frame future community-wide plans, and their revisions

  • Stewarding resources for collective impact, as appropriate

  • Providing logistical support for Continuum responsibilities as in Interim Rule โ€“ 24 CFR ยง578.7
