Indianapolis CoC Written Standards
The Indianapolis Continuum of Care (CoC) in coordination with the City of Indianapolis and homeless service funders have developed the written standards contained here for providing housing and services to those persons experiencing homelessness or at-risk of homelessness across Indianapolis. The goals of the written standards are to:
- Establish community-wide expectations on the operations of projects within the community
- Ensure that the system is transparent to users and operators
- Establish a minimum set of standards and expectations in terms of the quality expected of projects
- Make local priorities transparent to recipients and subrecipients of funds
- Create consistency and coordination between recipients' and subrecipients' projects within the Indianapolis CoC
Coordinated Monitoring Process
CHIP and the City of Indianapolis coordinate monitoring of CoC programs through a collaborative process. The types of monitoring, monitoring process, and roles between CHIP and the City of Indianapolis are outlined in the CoC Coordinated Monitoring Process document.
Program Models
The Indianapolis CoC utilizes program models to standardize practice across our homeless response system for programs and projects. At this time we are updating models, and as they are approved they will be posted here. We encourage agency partners, new service providers, funders and community members to review these models.
Coordinated Outreach
The Indianapolis CoC governing board approved an updated program model for Coordinated Outreach in October 2020. Coordinated Outreach is housing-focused street outreach that prioritizes engagement and relationship building to support access to permanent housing for individuals experiencing unsheltered homelessness. Outcome measures have been identified, and targets will be set.
Diversion
Homelessness diversion is a client-driven driven approach that helps individuals and families immediately identify safe alternative housing rather than entering shelter or experiencing unsheltered homelessness. More information about diversion can be found here.

Permanent Supportive Housing (PSH)
Permanent supportive housing is a model to address the needs of chronically homeless individuals. It combines affordable housing assistance with voluntary supportive services. More information about permanent supportive housing can be found here.

Rapid Rehousing (RRH)
The Indianapolis CoC governing board approved an updated program model for Rapid Rehousing in October 2020. This model was developed with the Rapid Rehousing Leadership Work Group, and aligns with HUD and best practices identified through the National Alliance to End Homelessness (NAEH).
